M. Niemann-Pick Typ C

Beim M. Niemann-Pick Typ C führt ein Defekt im NPC1-Gen (ca. 95% der Fälle) oder im NPC2-Gen (ca. 5 % der Fälle) zu einem gestörten intrazellulären Cholesterintransport und konsekutiv zu einer Akkumulation von unverestertem Cholesterin in den Zellen von verschiedenen Organen. Das Manifestationsalter reicht von der Perinatalperiode bis ins späte Erwachsenenalter. Die klinische Symptomatik ist sehr heterogen. Neben viszeralen Zeichen – bestehend aus einer Splenomegalie oder Hepatosplenomegalie- können neurologische Symptome, wie eine progressive zerebelläre Ataxie, Dysarthrie, vertikale supranukleäre Blickparese, Epilepsie, Kataplexie und eine progrediente Demenz imponieren. Zur Behandlung der progressiven neurologischen Symptome steht derzeit die Substratreduktionstherapie mit Miglustat zur Verfügung.
